Complex rotator cuff reconstructions

Superior capsular reconstruction, augmentation with grafts or biological implants and extensive capsulolabral reconstructions.

What it is
Some rotator cuff tears cannot be repaired directly: the tendon has retracted, lost quality or the muscle has atrophied. In these cases, reconstruction techniques restore the stability of the humeral head and reduce pain without resorting to a joint replacement.
When it is indicated
In massive or chronic tears, in failed previous repairs and in active patients who want to keep their own joint. The choice of technique depends on age, tissue quality and functional expectations.
How it is performed
Arthroscopically, a superior capsular reconstruction with a graft, augmentation of the repaired tendon with grafts or biological implants, or extensive capsulolabral reconstructions are performed, depending on the tear pattern.
Recovery
Sling protection for longer than after a standard repair, followed by staged physiotherapy. Graft integration sets the pace of strengthening.
